Part of my work as an occupational therapist is promoting client centred practice in health care. Advance Care Planning is one of the useful tools. I have posted this previously, but it’s good to revisit it occasionally.
Advance care planning (ACP) is the process of thinking about, talking about and planning for future health care and end-of-life care. It is about identifying what matters to you.
It isn’t just for elderly or already sick people. What would you like to happen if you had a bad accident or a stroke? You can identify your wishes – write them down or fill in an electronic form, let your family know and have this uploaded to the DHB website.
My father dies earlier this year and it was immensely helpful to know what his wishes were regarding end of life care and his funeral.
